Although pleural effusions may be seen in heart failure, the pulmonary markings on this x-ray are not increased. There are no findings of cephalization, interstitial nor alveolar edema to go along with this dx. Of note, findings on cxr consistent with CHF must be correlated with the clinical findings. For example, adult respiratory distress syndrome may have identical cxr findings as CHF but the clinical findings would distingish the diagnosis.
